Everything you need to know about our services, pricing, and what to expect at Check Up Sports Rehab.
Our Performance Physical Therapists use a hybrid rehab and training model. We combine injury treatment with training to prevent future issues — using evidence-based, individualized approaches tailored to your specific sport and goals.
Unlike traditional PTs, we don't just treat the symptom. We address the root cause, improve movement quality, and build performance so you come back stronger than before.
We are an out-of-network, cash-based practice and do not accept insurance directly. However:

Cash-based care puts your health first — not insurance requirements. Here's what that means for you:
Physical Therapy is for those currently recovering from an injury, pain, symptom, or surgery. The focus is on restoring function, reducing pain, and safely returning to activity.
Performance Training is for athletes and active individuals who are healthy but want to enhance performance, build strength, improve mobility, and prevent future injury. Think of it as proactive, not reactive.
Many clients progress from PT into Performance Training as they recover — and that transition is seamless with Dr. Mike.
